The ultimate consequence is severe damage to expensive fuel injection components due to water and particulate contamination.
Always prime the new filter with clean fuel before installation to minimize air entering the fuel system. Ensure the sealing gasket makes full contact and do not overtighten the filter housing.
Drain any accumulated water from the separator bowl regularly, and inspect the fuel lines for leaks when replacing the fuel water separator.
